Sub Acute Social Worker

Job Description


The Actors Fund Home, an assisted living, skilled nursing, accredited dementia care and subacute rehabilitation center located in Englewood, NJ, and a recipient of the U.S. News and World Report's coveted "Best Nursing Homes in America", seeks a Sub-Acute Social Worker who will be responsible for guiding patients and their families through the rehabilitation process in our 25 bed Sub-Acute unit. The Social Worker will coordinate admissions to The Actors Fund's sub-acute facility, provide clinical social work services during the patient's stay, and implement a safe discharge once services are completed.


         Work with patients and families to complete psycho-social assessments, admissions documentation, and case management once admitted.

         Schedule Careplan Meetings with patients and families.

         Participate in IDCP team meetings and Utilization Review.

         Communicate with department heads and finance team regarding insurance and financial arrangements.

         Arrange and implement a safe discharge plan, to home or elsewhere, as developed by IDCP team.

         Follow up with patients after discharge.


         May serve as Administrator on Duty as needed.

         May be asked to cover for Director of Social Services or Director of Admissions.


         Masters of Social Work degree.

         Current New Jersey state license, LSW or LCSW.

         5 years' experience in sub-acute and/or acute care.

         Experience working with seniors preferred.

         Excellent written, verbal, and interpersonal communication skills.

         Experience with managed care/insurance and public payer sources.


This is a full-time salaried position based in The Actors Fund Home in Englewood, New Jersey. Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
